At a minimum you need a have an ESSR for each formal placement you have undertaken since your last ARCP. You are also expected to regularly meet with your Educational Supervisor for discussions during your placement. Please note that you do NOT need to complete a form R and an MSF for EACH placement: you only require 1 complete MSF and 1 up-to-date form R for an ARCP.
