Why do I need to complete a Form R?

Completion of a Form R each year is mandatory. This form holds your up‑to‑date demographic information and includes the self‑declaration required for revalidation. It also provides an opportunity to review, update, and correct your details, which supports the renewal of your registration with NHS England.


Form R (Part B) is specifically required as part of the evidence for your ARCP and revalidation. You must complete a new Form R (both parts) every time an ARCP is held for you, including during any Out of Programme (OOP) placements.
